COMPLAINT CONTROL NUMBER: 24-AS-20260526102722

Facility gate is in disrepair causing a safety hazard.

Licensing Program Analyst (LPA) Sarah Hurt arrived at the facility unannounced to investigate the allegations listed above. LPA explained the purpose of the visit. LPA was granted entry by Administrator Nancy Cudal.

Regarding the allegation Facility gate is in disrepair causing a safety hazard. The facility front gate latch does not close correctly. however this does not cause a safety hazard to facility residents. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
