Facility Evaluation Report

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to Conduct a Case Management visit. LPA met with facility Administrator Nancy Cudal, and explained the purpose of today's visit.

During the inspection, LPA observed the exterior metal gate was in disrepair. The gate's latching mechanism appeared damaged and did not function as intended. Rust and deterioration were observed on the gate hardware.Although the main facility entrance door remained operational and secured the building, LPA

discussed with facility staff the need to repair or replace the gate and maintain the facility in good repair.

LPA observed facility residents utilizing wire to secure the facility main door open. LPA discussed removal of the wire and recommended facility staff install an appropriate device to hold the door open.

Technical Violation reviewed, Exit interview conducted with facility Administrator Nancy Cudal, and a copy of this report along with appeals rights provided.
