Lack of care and supervision resulting in AWOL
Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to investigate the above allegations. LPA met with facility facility staff Nancy Cudal, and explained the purpose of today's visit.
Regarding the allegation Lack of care and suprevision resulting in AWOL. Resident 1 is able to leave the facility unsupervised. Resident 1 did leave the facility on 03/20/2026, and returned on 03/21/2026. The facility informed Resident 1's Responsible party, and the local police. The facility also offered Resident 1 to be seen by a doctor upon their return, to which they declined. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
