Facility Evaluation Report
Licensing Program Analyst (LPA) Katie Brown arrived unannounced to conduct a Health & Safety Inspection in conjunction with an Initial complaint Visit (24-AS-20260313093332). LPA met with, explained the reason for the visit with Facility Designee Oscar Chavez (FD) and Med Tech Rowena Koster (MT).
During this visit, LPA Brown, FD and MT toured the facility inside and out. During the facility tour, LPA Brown selected resident rooms and bathrooms to observe. Rooms contained required furnishings, linens, hygiene items and an air individual conditioner. Bathrooms were found to be clean and contained required items.
The kitchen was toured and found to be clean and safe for food preparation. LPA observed adequate perishable and non-perishable food supply also including refrigerator and freezers storing food. Knives/sharp items are locked in the kitchen and inaccessible to residents.
Doors and passageways were clear throughout the facility. Fire extinguishers were serviced by Fire System Solution on 2/5/2026. Medications are centrally stored in a Medication Room which is where a First Aid Kit is kept. Cleaning/disinfecting supplies and chemicals and extra paper goods and supplies are locked and stored separately from any food. LPA walked outside to find the grounds well-kept with clear walkways with sitting areas.
Deficiencies are being cited in accordance with the California Code of Regulations on the attached LIC 809-D
An exit interview was conducted and Plan of Correction (POC) developed. A signed copy of this report and Appeal Rights were provided.
