Complaint Investigation Report

Based on interviews and records review, staff contacted emergency services for R5. Incident reports indicate R5 was sent to the hospital.

Based on interviews, records review and observation, Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

An exit interview was conducted with facility Administrator Nancy Cudal and a copy of this report was provided.
