Administrator Ria Loria and Staff Nancy Cudal

Staff not providing adequate food service.

Licensing Program Analyst (LPA) Shawna Doucette conducted an unannounced complaint visit and was granted entry by Staff Nancy Cudal. Administrator Ria Loria responsed to the facility and met with LPA. LPA discussed the purpose of the visit.

LPA conducted interviews with staff and residents and reviewed residents file. LPA toured the facility and took photos of food supply and menu.

Based on record review and interviews, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. Per California Code of Regulations, Title 22, Division 6, Chapter 8, a deficiency is being cited on the attached 9099-D.
