Facility Evaluation Report
On 2/202/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Inspection. LPA introduced self, stated purpose of visit and allowed entrance. LPA met with Administrator, Christine Espinoza to conduct tour.
Day Program has 4 different buildings on site. Participants begin their day on site and then are community based, at the conclusion of day, participants return to site before being transported to their home.
Tour began in Suite A, this building is contains both offices and meeting area. Interior of building observed to be clean and in good repair. LPA observed the sprinkler valve near front entrance to be leaking water, and observed the flower bed to be flooded. Water temperature measured at 116 degrees F.
Suite B contains 1 office and 2 rooms which are designed to provide services to four (4) participants. LPA observed interior of building to be clean and in good repair. Water temperature measured at 120 degrees F.
Suite C contains 3 classrooms to provide services to nine (9) participants. LPA observed the following on exterior, water faucet near front door observed to be dripping and area beneath faucet observed green and a potential slip hazard. Faucet in bathroom sink is dripping water. Water temperature measured at 120 degrees F.
Suite D contains 1 office and 3 classrooms serving nine (9) participants. LPA observed the light fixture in classroom 2 to be missing 1 light bulb and observed to have an open socket. Water temperature measured at 109 degrees F.
Each suite has a designated area for all sharps to be locked and secured. Each suite has an individual first aid kit and observed to have all required items. LPA observed all carbon monoxide detectors and smoke detectors in each suite to be operational during inspection. Each suite observed to have a fire extinguisher with a service date of 10/01/2024. Day Program does not prepare meals on site, participants bring meals from home or eat in the community. No medications are stored or administered on site or during day program hours.
