Facility Evaluation Report
On 02/17/22, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an Annual Inspection - Infection Control. LPA introduced self, stated the purpose of the visit and requested to meet with the Administrator. LPA met with program assistant Lori Reed. Administrator Meghan Holland was called. 11 clients were present during the inspection. LPA completed a tour of the facility with program assistant and Administrator arrived during tour.
Visitor log-in/temperature check was observed upon entry. Hand sanitizer was readily available to clients and visitors. The Day Program has 6 classrooms, 3 on each side. Facility is maintained at a comfortable temperature and no passageway obstructions or fire hazards were observed inside or outside. Client restrooms were tour, observed to be clean, and operational. LPA observed hand washing posting by all sinks. Social distancing is maintained in the common and dining areas. LPA observed social distancing and cough etiquette postings in facility. LPA observed 30-day PPE supply.
Clients bring their own lunches to program.
There is a program outside area free of obstruction. Clients’ cubbies were observed in the classrooms. Facility dining area toured. Lighting and seating appear to be adequate throughout the facility. A sample of client and staff's files were also reviewed to have updated emergency contact information. Staff files have current 1st Aid and infection control training. Fire extinguisher was observed with a service date of: 08/27/21. Last fire drill was 01/31/22.
Exit Interview conducted. The following documents are requested and submitted to Fresno CCL by: 11/08/21. The following updated forms were requested: Lic 308, Lic 309, Lic 500, Lic 610D, and Lic 9020. Administrator was informed that as COVID-19 precautionary measure, this report will be provided via email. Signed report on file.
