Facility Evaluation Report
On 3/25/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Case Management visit regarding a self reported AWOL that was received in Fresno Regional Office on 3/24/2026.
LPA met with Regional Supervisor, Daniel Esparza and Administrator, Blanca Arteaga. during today's visit.
Per incident report received in Fresno Regional Office, Resident 1 (R1) AWOL'd from the facility on 3/23/26. Incident report documents R1 was on a neighbor's property attempting to gain access to their backyard.
During interviews and review of documentation, it is documented that R1 did leave facility however staff were present within a safe distance. Staff remained following R1 until R1 was able to be re-directed and return to facility with staff.
LPA received copies of IPP and staff documentation during case management visit.
No deficiency cited during visit. Chapter 8.
Exit interview conducted. A copy of report and appeal rights provided to Administrator.
