Facility Evaluation Report

On 05/03/2022, Licensing Program Analyst (LPA) L. Salazar arrived at the facility unannounced to conduct a Health & Safety Inspection. LPA explained the purpose of the visit with Licensee Tiffany Apple. Covid precautionary measures were taken at the time of entry.

LPA toured the facility inside out and observed a 3 bedroom 2 bathroom house. LPA observed 2 out 2 residents in care. LPA observed 2-day supply of perishable and 7 day nonperishable food supply. Facility inside temperature is set at 71. LPA observed living areas and personal areas to be clean, free from clutter and debris.

No deficiencies were cited during this visit. An exit interview was conducted with the Licensee
