Facility Evaluation Report
On July 9, 2026 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced. LPA was allowed entrance into the facility and met with Administrator Karina Hernandez .
The Dept previously received an incident report regarding C1 being left in a transportation van for about 20 minutes. LPA spoke with Administrator who stated staff are trained when they are first hired and every 6 months after. Facility conducts monthly meetings and has now implemented transportation safety to monthly meetings.
Citation issued for care and supervision and civil penalty has been issued to the facility regarding the incident.
Administrator provided verification of training that was provided to staff after the incident. Copies were provided to LPA.
Exit interview was conducted and a copy of this report LIC809, LIC809D, LIC421IM, and appeal rights were provided to Administrator Karina Hernandez.
