Facility Evaluation Report
On 11/30/2023, Licensing Program Analyst (LPA) Walton arrived for an announced prelicensing inspection. LPA introduced self, stated the purpose of the visit and requested to meet with the Administrator. LPA met with Administrator, Karina Hernandez.
LPA conducted a facility tour. The facility has a fire clearance for 70 ambulatory and 5 non-ambulatory, for a total capacity of 75. No fire hazards or passageway obstructions were observed. The facility has 7 activity rooms . Facility has a quiet room for clients as needed. LPA observed adequate lighting throughout the facility. LPA toured the client restrooms which appeared clean and operational. Hot water measured at 133.2 degrees F, Administrator adjusted the facility water temperature. LPA and Administrator waited approximately 20 minutes and measured the water again, hot water measured at 101.7 degrees F. Clients will provide their own meals and the facility will provide snacks to clients as needed. Facility will utilize activity rooms six and seven as a dining area when needed. LPA observed a locked closet for chemicals. Fire extinguisher service date: 11/6/2023 . Fire alarm and carbon monoxide detector tested and observed to be operational during today's inspection.
This facility is being licensed as a change of location. Per Administrator, all furnishings postings and storage lockers will be transferred to this location once the license for the above facility has been obtained. The Department will conduct an unannounced inspection, once the license is received, to follow up and ensure all furnishings, postings and storage has been transferred to the above facility
Component III was conducted during today’s pre-licensing visit.
LPA observed the following correction needs to be made:
Bring the hot water temperature within range between 105-120 degrees F.
LPA will notify CAB that facility is ready to be licensed once the above correction has been made.
