Facility Evaluation Report

On 03/11/26 Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an annual visit. LPA introduced self, stated the purpose of the visit and was greeted by staff Sean Ryan. LPA was granted entry. LPA met with House Manager Shellie Ryan. Administrator Ronald Duran was called and arrived later during inspection. LPA toured facility with S1. Two clients were present during inspection.

The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. An adequate supply of perishable and non-perishable food was observed. Extra linens and towels were observed. Fire extinguisher was observed with a service date of: 08/22/25. Fire drill last completed: 02/2026. Medications observed kept locked under hall counter. Cleaning supplies and chemicals stored and locked in chemical closet and laundry room.

All bedrooms were observed to have the required furnishings and adequate lighting. The bathroom was properly equipped and operating. Hot water temperature was tested at 116.7 and 116.6 degrees F in bathroom 1 and 115.3 and 113.6 degrees F in the bathroom 2. Outside of facility toured and observed to be free of debris. Adequate outdoor seating observed available for clients. Side gate observed self-closing. Carbon monoxide and smoke detectors were tested and observed to be operational. All clients’ and sample of staff files were reviewed to have all the required documents. Client’s P & I was checked.

A deficiency is being cited on the attached Lic 809D in accordance to California Code of Regulations, Title 22, Division 6.

Exit Interview conducted. The following documents are requested and submitted to Fresno CCL by: 03/16/26. Forms requested: Lic 308, Lic 610D, and Administrator certificate. A copy of this report was provided to Administrator whose signature on this form confirms receipt of this report.
