Facility Evaluation Report

On 03/27/25 Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an annual visit.

introduce self, stated the purpose of the visit and was greeted by staff Sean Ryan. LPA was granted entry. LPA met with House Manager Shellie Ryan. Administrator Ronald Duran was called and arrived shortly. LPA toured facility with Administrator. All three clients were present during inspection.

The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. An adequate supply of perishable and non-perishable food was observed. The temperature is maintained for refrigerator at 36 degrees F and freezer at -8 degrees F. Fire extinguisher was observed with a service date of: 08/13/24. Fire drill last completed: 01/24/25. Medications observed kept locked under hall counter. MARs were reviewed. Cleaning supplies and chemicals stored and locked in chemical closet. All bedrooms were observed to have the required furnishings and with adequate lighting. The bathroom was properly equipped and operating. Hot water temperature was tested at 114.3 and 114.4 degrees F in bathroom 1 and 115.3 and 113.7 degrees F in the bathroom 2. Outside of facility toured and observed to be free of debris. Adequate outdoor seatings observed available for clients. Side gate observed self-closing. Carbon monoxide and smoke detectors were tested and observed to be operational. All clients’ and sample of staff files were reviewed to have all the required documents.

No deficiencies issued during this inspection.
