The inspector’s account

On 07/06/26, Licensing Program Analyst (LPA) M. Yang conducted case management incident visit to the facility. LPA introduced self, stated the purpose of the visit, and met with Licensee/Administrator Socorro Ann Telmo.

The purpose of the visit is to conduct a health and safety visit on the resident in care and follow up on written

incident report that was reported to the department where staff observed R1’s right ankle and left knee swelling with no prior fall incident.

Interviews were conducted and records were received. The information provided will be reviewed; a follow up case management will be conducted if necessary for the incident report.

During tour of facility, R1 was observed laying in bed using full rails. A deficiency is being cited, per California Code of Regulations, Title 22, Division 6, see attached Lic 809D.

An exit interview was conducted. A copy of this report and appeal rights were provided to Licensee, whose signature confirms receipt of this report.
