Facility Evaluation Report
On 12/22/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an annual visit. LPA introduce self, stated the purpose of the visit and met with Administrator Victoria Doyle. No client was present during inspection. LPA toured facility with Administrator.
The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. An adequate supply of perishable and non-perishable food was observed. Refrigerator temperature is maintained at 33 degrees F and freezer at 0 degrees F. Cleaning chemicals and sharps observed stored and locked in facility office. Medications were observed locked in hall closet. MARS were reviewed and medications were checked.All bedrooms were observed to have the required furnishings and adequate lighting. The bathrooms were toured. Hot water temperature was tested at 106.3 and 105. 2 degree F in suite bedroom bathroom, 110.8 and 105.2 degree F in hall bathroom, and 108.9 degree F in bedroom bathroom. Fire extinguisher was observed in laundry room and hall with serviced date of 12/09/25. Extra linens and towels were observed. Outside of facility toured and observed to be free of debris. Pool fence was observed locked and secured. Adequate outdoor seating is available for clients. Side gate was observed free of obstruction. Smoke detectors and carbon monoxide were observed operational during visit. Clients’ and staff files were reviewed to have all required documents. Clients P & I were reviewed and checked.LPA discussed with the Administrator the outstanding Facility Annual and late fees.
Exit Interview conducted. The following documents are requested and submitted to Fresno by 12/29/25. Lic 308, Lic 500, Lic 400, Lic 402, Lic 610D, Lic 9020, and current Administrator certificate. A copy of this report was provided to Administrator, whose signature confirms receive this report.
