Licensing Program Analyst (LPA) Shawna Doucette arrived at the facility to conduct a case management regarding an incident that occurred on 7/2/24. LPA was granted entry by Staff Xinia Ulloa. LPA met with Regional Directior Latisha Albritton.

LPA conducted staff and client. LPA obtained a copy of facility's internal investigation.
