Licensing Program Analyst (LPA) Shawna Doucette arrived at the facility to conduct a case management inspection regarding an incident that was reported on 04/30/24. LPA met with Office Staff Xinia Ulloa.
LPA asked for C1 and C2's records. Office staff stated clients do not attend this day program.
