On 06/23/26, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct a case management deficiency visit. LPA met with Administrator Christina Pittman.
LPA discussed SOC 341 that was received by the department on 05/22/26. Staff 1 (S1) have spoken inappropriately to clients while providing care and supervision on two different occasions.
A deficiency is being cited on the attached Lic 809D in accordance to California Code of Regulations, Title 22, Division 6.
An exit interview was conducted. A copy of this report and appeal rights was provided to Administrator, whose signature on this report confirms receipt of report.
