Facility Evaluation Report

Licensing Program Analyst (LPA) Shawna Doucette conducted an unannounced Case Management for an incident that occurred on 12/2/25. LPA met with Program Director Christina Pittman.

LPA interviewed Program Director who stated S1 was accused of spending $2.70 of C1's money on 12/2/25. Program Director stated the facility conducted an investigation and confirmed the accusation. Program Director stated C1's money was reimbursed on 12/8/25. Facility immediately put S1 on leave pending the investigation. S1 was terminated once investigation was complete.

A copy of this report was provided to Program Director with appeal rights and plans of correction.
