The inspector’s account

On 7/09/2025, Licensing Program Analysts (LPAs) M. Medina and J. Duarte conducted an unannounced Annual Required Inspection. LPAs arrived, introduced themselves, stated purpose of visit and allowed entrance. Program Director, Victoria Green was contacted and conducted inspection with LPAs.

Facility toured with Program Director. Day Program observed to be clean, odor free, and a comfortable temperature. LPAs toured an Art room, sensory room which is also utilized as quiet room, movie room, computer room, and book room during day program tour. LPAs observed clients to be engaged in various activities with staff throughout facility. Day program has 2 separate kitchens, that are utilized for cooking experiences, no meals are prepared on site for clients. Clients bring their lunch from home for day program. Medications observed to be locked and secured in the kitchen cabinet. LPAs observed medications are being administered as prescribed and MARS is being utilized according to regulation. Client restrooms toured, all fixtures observed to be operational at time of inspection. Water temperature measured between 108 and 115 degrees F.

Facility extinguishers present throughout facility with a service date of 4/01/2025. Facility is equipped with pull stations, strobe lights, sprinklers throughout facility. Carbon monoxide detectors and smoke detectors present and observed operational at time of inspection. First Aid kits present throughout facility and observed to have all required items.

No deficiencies cited. Exit interview conducted and a copy of report provided for day program records.
