05/05/2022 and conducted by Evaluator Kelly J. McClurg

COMPLAINT CONTROL NUMBER: 24-AS-20220505123625

Manager-Member /Administrator (Admin) Socorro "Ann" Telmo; Manager-Member (MM) Dio Telmo;

A Complaint visit was conducted on the date & during the times indicated above by Licensing Program Analyst (LPA) K. McClurg. LPA met with Manager-Member /Administrator (Admin) Socorro "Ann" Telmo & Manager-Member (MM) Dio Telmo. LPA stated purpose of visit & reviewed allegation.

It was determined that Resident 1 (R1) was not accepted back from hospital by facility. No eviction notice provided to R1 &/or responsible party.

The Department has investigated the above allegation & determined it to be substantiated.

Exit interview conducted with Admin & MM. Report provided.
