Facility Evaluation Report

On 5/15/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection visit. LPA arrived, stated purpose of visit, and allowed entrance by Administrator, Jhosan Timbreza.

LPA conducted facility tour with Administrator. There are 4 residents in placement at this. All residents attend day program Monday through Friday. Facility tour conducted with Administrator. Facility observed to be clean odor free, and maintained at a comfortable temperature. Facility tour began in resident bedrooms. All bedrooms observed to have required furnishings. Resident bathrooms toured, water temperature measured at 113 degrees F. Shower observed to have a shower chair and skid resistant mat available. All fixtures observed operational. Kitchen toured, facility observed to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available. LPA observed kitchen to have all sharps and utensils locked and secured in kitchen drawer. Medications are centrally stored and locked in kitchen cabinet. Medications reviewed for residents, all medications observed to have original labels and to be administered as prescribed.

Fire extinguisher present with a service date of 06/23/2025. . Smoke and Carbon Monoxide detectors present and observed operational during inspection. Last Fire drill conducted 4/1/426 according to facility records.

LPA reviewed resident files, staff files, P & I records and Disaster Binder.

Outside of facility toured. Facility has an outdoor shaded seating area available for residents. Exits open free of obstruction. No hazards observed.

LPA received thee following documents during inspection: Administrator Certificate, LIC 500 (Personnel Report), LIC 610D (Emergency and Disaster Plan) and LIC 9020 (Register of Facility Clients/Residents).

No deficiencies were cited during this inspection. Exit interview was conducted and a copy of this report provided for facility records.
