On 09/26/2025, Licensing Program Analyst (LPA) J. Duarte conducted an unannounced annual visit and addressed a Case Management. LPA met with Administrator, Myra Celeste Castillo to discuss incident report.
LPA followed up on a self reported incident that department received on 07/24/25, that occurred on 07/21/2025, and involved Resident 1 (R1) and Resident 2 (R2).
LPA gathered additional information for incident from Administrator and obtained copies of R1 and R2 physician reports.
No deficiencies cited during case management visit..
