Facility Evaluation Report
On 5/29/25, Licensing Program Analyst (LPA) M. Medina conducted an unannounced required Annual Inspection. LPA introduced self, stated purpose of visit and allowed entrance. LPA met with Administrator, Harold Dano to conduct facility visit.
LPA observed facility to be clean, odor free, and well lit. Facility tour conducted with Administrators, tour began in resident bedrooms. Bedrooms observed to have all required furnishings, linen observed to be in good repair with additional linens available. Resident bathrooms toured, all fixtures observed operational. LPA observed grab bars, shower chairs and skid resistant mats available. Water temperature measured at 109 degrees F. All common areas observed to have adequate seating available. Kitchen toured, LPA observed facility to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available. All knives are locked and secured in kitchen. Medications observed to be locked and secured in kitchen cabinet. Medication observed to have original labels and to be administered as prescribed.
Fire extinguishers present with a purchase date of 3/16/2025. Carbon monoxide and smoke detectors present and observed operational during today's inspection.
Outside of facility toured, all exits open free of obstruction. Sheds in the backyard observed to be locked, secured, and inaccessible to residents.
Staff and resident files reviewed. LPA received packet for change of Administrator for Christina Marsy during facility inspection,
Exit interview conducted and a copy of report provided for facility records.
