Facility Evaluation Report
On 1/29/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection. LPA introduced self, stated purpose of visit, and allowed entrance by Direct care staff. Licensee/Administrator Ofelia Nebrida also present at time of LPA arrival.
There are currently five (5) residents in care. Three residents present at time of visit, two residents were at day program. Residents observed to be relaxing in their bedroom, and some to be engaged in activities in the living room with staff. Facility tour conducted with Administrator, facility observed to be a comfortable temperature, clean, odor free, and well lit. Facility has 5 private bedrooms, all bedrooms observed to have required furnishings available. Bathrooms toured, showers observed to have shower chairs, grab bars, and skid resistant mats available. Toilet area also observed to have grab bars. Water temperature measured in bathroom at 115 degrees F. Kitchen toured, facility observed to have a 2-day supply of perishable food and a 7-day of non-perishable food available for residents in care. All sharps observed to be locked and secured in kitchen drawer. Medications observed to be locked and secured in kitchen cabinet. Medications observed to have original labels and to be administered as prescribed.
Facility and staff files reviewed during inspection, Fire extinguisher present with a service date of 6/23/2025. Carbon monoxide detector and smoke detector observed operational during inspection. Last fire drill conducted on 12/23/25 according to facility records.
Outside of facility toured. Facility has sufficient seating and a shaded patio area available for all residents. Emergency exits open free of obstruction. No hazards observed. LPA received a current copy of liability insurance during inspection.
No deficiencies observed during inspection.
