On 5/29/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Case Management. LPA met with Administrator, Ofelia Nebrida to conduct visit.
LPA conducted visit to follow up on a self reported incident for R1 that occurred on 05/09/25 where are R1 had an unobserved fall. LPA conducted interview and obtained additional documentation during case management visit. .
R1 was observed to be seated in recliner participating in day program activity with staff observing.
