Facility Evaluation Report

On 12/12/25, Licensing Program Analysts (LPA) J. Duarte and Licensing Program Manager (LPM) A. Walton, conducted an unannounced Case Management visit for the purpose of Health and Safety of the resident in care. LPA contacted Administrator (AD) Dinafay Crandell via telephone and she arrived shortly after. LPA and LPM introduced selves, stated the purpose of the visit and were allowed entrance. LPA toured the facility with AD.

AD stated that the resident is on a community outing.

LPA toured the kitchen and observed a two-day supply of perishable and a seven-day supply of nonperishable food. LPA toured the residents’ rooms and observed rooms to have required furnishings. All common areas observed to have adequate seating available for residents in care.

LPA is requesting the following documents for R1 to be submitted to the Fresno CCL office by 12/15/2025: physician report, IPP, emergency ID, Med list/ Mars for November and December of 2025, Admission agreement, and charting notes/narratives.

An exit interview was conducted with AD and a copy of this report was provided.
