The inspector’s account

On 2/21/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required inspection. LPA arrived, introduced self, stated purpose of visit, and allowed entrance by Direct Support Professional. Administrator, Jhosan Timbreza contacted by telephone and arrived a short time later to conduct inspection. Currently, four residents in care. At time of LPA arrival all residents had been picked up by transportation to attend day program. Residents attend day program Monday through Friday 8:00 AM to 2:00 PM.

Facility tour conducted with Administrator. Facility observed to be clean, well lit, and odor free. All common areas of the facility observed to have adequate seating available for residents. Resident bedrooms toured, and all observed to have required furnishings available, each resident bedroom has fire exit. Resident bathroom toured, all fixtures observed operational. Water temperature measured at 107 degrees F. Kitchen toured, facility observed to have a 7-day supply of non-perishable food and a 2-day supply of perishable food available. All knives observed to be locked and inaccessible in kitchen drawer. Cleaning supplies are locked and secured under kitchen sink.

Smoke and carbon monoxide detector observed operational and mounted in the hallway. Fire extinguishers present and have a service date of 7/01/2024. Last fire drill conducted on 12/18/2004 according to facility records.

Outside area toured. All exits open free of obstruction, no hazards observed.

Staff and resident files reviewed. LPA requested the following items to be submitted to Fresno Regional Office no later than Friday 3/07/2025: LIC 500 (Personnel Report) and LIC 9020 (Register of Facility Clients/Residents).

No deficiencies observed. Exit interview conducted and a copy of this report provided for facility records.
