Client consumed illegal drugs on the premises

On 9/22/23, Licensing Program Analyst (LPA) M. Medina conducted a subsequent complaint visit to deliver findings to complaint. LPA met with Ofelia Nebrida, Administrator to deliver findings.

This Department investigated the complaint allegation, during the course of the investigation, records were reviewed and interviews were conducted. Based on interviews and records reviewed there is not a a preponderance of evidence to prove or disprove the allegation occurred therefore the allegation is UNSUBSTANTIATED.

No deficiencies cited. Exit interview conducted with Administrator and a copy provided for facility records.
