Facility Evaluation Report

On 6/18/2025, Licensing Program Analysts (LPAs) J. Duarte and M. Medina conducted an unannounced Case Management visit. LPAs arrived, introduced selves, allowed entrance by Direct Care Staff. Administrator, Ronald Duran contacted by telephone and arrived a short time later to conduct facility visit.

LPAs arrived to follow up on a self reported incident that occurred on 6/11/25 when Resident 1 (R1) left facility unattended through bedroom window about 6:00 pm, R1 is a 1:1 with 8 hours per day. Staff heard window chime, and when resident rooms were checked R1 did not respond and door was locked. Staff check exterior of facility and noticed R1's window was open and screen was off, and R1 was not present. R1's physician report (LIC 602) indicates that R1 is unable to leave facility unattended.

Bakersfield Police Department was notified, and R1 was located by Bakersfield Police Department and returned to facility at approximately 9:40 PM.
