Licensing Program Analyst LPA Shawna Doucette arrived at the facility unannounced to conduct a case management regarding an incident that occurred on 4/14/25. LPA met with Administrator Anthony Arambula.
During the course of the investigation, it was found the incident occurred at another facility involving a client that resides at this facility.
