Facility Evaluation Report
On 06/26/2026, Licensing Program Analyst (LPA) M Vega arrived at the facility, unannounced to conduct a case management visit based on a self-reported incident. LPA met with Assistant Administrator - Wendy Gaspar.
The facility has submitted Special Incident Reports (SIRs) to CCL reporting Client 1 (C1) elopement from the facility – C1 was under the care and supervision of facility staff while outside of facility. Staff was following C1 closely. LPA obtained LIC 602 and supporting documents.
There were no deficiencies or citations issued during this Case Management.
An exit interview was conducted, and a copy of this report was signed and provided to Assistant Administrator for facility records.
