Facility Evaluation Report

On 4/09/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Case Management visit. LPA arrived, introduced self, stated purpose of visit and allowed entrance by Administrator, Sandra Maldonado,

LPA arrived to follow up on a few incident reports received in Fresno Regional Office on the following dates, 2/26/2026.

It was reported on 2/26/2026 that resident 1 (R1) was dropped off early by day program transportation without staff presence at facility. Day program reported to facility that due to individuals being absent from day program, transportation route was shortened. Facility was not notified prior of drop off time change and no staff were present. Day program staff did not verify if facility had staff present before leaving R1 outside perimeter fence and there is no visual of front door.

Sheriff Department was notified when staff realized R1 had been dropped off and was not present at facility, R1 was located by staff within 10 minutes. Sheriff's department conducted a wellness check later that evening and a report number was provided.

Administrator stated that after incident occurred, it was requested that day programs drive into round about driveway so that staff are aware of resident drop offs.

Exit interview conducted and a copy of report will be provided via e-mail for facility records.
