Licensing Program Analyst (LPA) Shawna Doucette arrived at the facility unannounced to conduct a case management for an incident that occurred on 4/14/25. LPA met Administrator Jose Marquez.
During the course of interviews, it was found staff from this facility sent a photo of a client (C1) who resides at another facility to a parent at this facility that has no relation to the client. The photo was of the client's injuries from a previous abuse allegation.
A copy of this report was provided with plans of correction and appeal rights to the Administrator.
