Facility Evaluation Report
On 4/28/2025, Licensing Program Analyst (LPA) Rachel Bruce conducted an unannounced case management visit at the above facility . LPA met with licensee Jose Valdez. (JV), introduced herself, explained the nature of the visit. LPA was provided a tour of the facility. Present were 2 staff and 3 residents.
This visit was prompted by an SOC341 submitted on 4/24/2025. The report was filed due to an alleged altercation between staff and resident. (See confidential names list attached). The report states that on 4/23/2025 R1 grabbed staff's necklace (S1) who then proceeded to allegedly kick R1 in the ribs. At that time, S2, S3 and S4 were physically restraining the resident.
LPA reviewed both staff and resident files of those involved. LPA also conducted phone interview with Erenique Jones, Administrator. LPA obtained copies of pertinent information from both personnel files and residents.
An exit interview was conducted with JV. Further investigation is needed. LPA will return to the facility for further action if necessary.
On 5/28/2025 Licensing Program Analyst (LPA) returned to the facility to make the correction to the document reflecting that no citation has been issued as the preponderance of evidence standard has not been met. The matter will be closed as of today's visit. No follow up needed. Since the alleged incident, staff have received refresher training and the Administrator has assured that staff will be closely monitored and reminded of how to appropriately handle incidents requiring restraint in order to avoid allegations of this nature.
