Facility Evaluation Report

Licensing Program Analyst (LPA) Daiquiri Boyd conducted an unannounced visit today for the facility’s annual inspection. LPA arrived at the facility at 9:45 am and found no one to be at the facility. LPA waited for 30 minutes and called another SAILS facility in the area and was able to speak to administration and get the phone number of Administrator Bertha Marquez, who arrived at the facility a short time later with Lead Staff, Ana Tinoco. LPA met with facility Administrator, Bertha Marquez. Administrator's Certification for Bertha Marquez expires June 23, 2026. There are currently 4 residents who reside at this home. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, garage and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable.

Fire extinguisher was serviced on 3/27/25. Smoke alarms were tested and are operational. The home has a carbon monoxide detector, which was tested, and performs disaster drills as required. Water temperature was tested at 112.6 degrees. First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible. LPA Boyd reviewed 3 staff files, and 4 client files. LPA Boyd confirmed all staff present is background cleared.

There were no deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22.
