On 11/10/21 at 11:35 AM, Licensing Program Analyst (LPA) Malia Thao arrived unannounced to conduct a case management - other inspection. LPA met with Rochelle Balaban, Administrator and Faith Enriquez, Lead LVN.
LPA amended Plan of Correction for deficiency issued on 10/18/21.
No deficiencies cited during the inspection.
