Facility Evaluation Report
On 12/5/2025, Licensing Program Analyst (LPA) B. Miranda arrived unannounced to conduct an annual inspection. LPA introduced self, stated the purpose of the visit and was granted entry to the facility by the Administrator Leslie Richard.
Per Licensee, the facility has not accepted clients and the census is zero. The facility does not have staff at this time. LPA conducted a tour of the facility with Administrator. Facility has adequate seating and lighting available. LPA observed required furnishings for resident bedrooms. Facility has 7 days worth of non-perishable and 2 days worth of perishable food items. Kitchen is clean and safe for food preparation. Resident bathroom was clean and operational. Facility has locked cabinet for medications and knives.
No fire clearance issues observed during today's inspection. Kitchen water temperature was checked and read at 108.8 degree Fahrenheit. Smoke and carbon monoxide detectors are in working order. Fire extinguisher was purchased on 12/4/25.
Administrator will notify the Fresno CCL office once the facility accepts it's first resident. Administrator will be in compliance with Title 22 when first resident has moved into the facility.
