On 12/13/2022, Licensing Program Analyst (LPA) Walton attempted to conduct an unannounced annual inspection. LPA knocked on the door and rang the doorbell and did not receive a response. LPA contacted the facility number on file and was unable to leave a message.
LPA waited approximately 20 minutes for facility staff to arrive.
LPA will return at a later date to conduct this inspection.
