Facility Evaluation Report

Licensing Program Analyst J. Duarte arrived at the facility announced to conduct a case management for facility closure. LPA met with Licensee/Administrator Mukesh Patel and DR. Pierre Barnes.

During a Non-Compliance Conference (NCC) on 07/17/2026, the Licensee voluntarily chose to relinquish the license. LPA toured the facility. There were no residents observed to be residing in the facility. LPA requested and obtained the facility license.

An exit interview was conducted, and a copy of this report was provided .
