Facility Evaluation Report

Licensing Program Analyst (LPA) M Vega conducted an unannounced Case Management visit. LPA met with facility staff, Shawn Walton, and explained the purpose of today’s visit.

LPA interviewed Resident 1 and confirmed Resident 1 is safe.

LPA discussed Resident 1’s (R1) placement with facility staff.

LPA also confirmed that no written 30-day eviction notice has been issued to R1. The licensee was advised that R1 shall remain in placement and continue receiving required care and supervision until any discharge or eviction is completed in accordance with applicable Title 22 requirements.

Exit interview conducted with facility staff, Shawn Walton, and a printed copy of this report was provided for facility records.
