The inspector’s account

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced Case Management visit. LPA met with facility staff, Shawn Walton, and explained the purpose of today’s visit.

LPA interviewed Resident 1 and confirmed Resident 1 is safe.

LPA reviewed Resident 1’s Physician’s Report and confirmed the assessment documents that Resident 1 requires care and supervision.

LPA discussed Resident 1’s placement with facility staff. LPA advised that, based on the current medical assessment, Resident 1 requires care and supervision and shall remain in the licensed facility where services are provided by fingerprint cleared facility staff under the licensee’s responsibility.

LPA also confirmed that no written 30-day eviction notice has been issued to Resident 1. The licensee was advised that Resident 1 shall remain in placement and continue receiving required care and supervision until any discharge or eviction is completed in accordance with applicable Title 22 requirements.
