Facility Evaluation Report

On April 15, 2026 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to conduct a case management visit regarding an incident that was report to the Dept on April 9, 2026. LPA met with Cheryl McCraw Administrator and explained the reason for the visit. LPA was allowed entrance into the home.

LPA observed one resident at the facility and the other resident was at day program. LPA observed facility to be clean, clutter free, and odor free.

LPA reviewed MARs and centrally stored medication log which are complete and up to date. Facility is currently conducting their own investigation regarding a medication error. Facility verified R1's medication for Oxcarbazepine 600MG was missing for April 9, 2026 and was already given for the date of April 8, 2026. Facility was not able to provide current medication training for S1.

Deficiencies observed during today's visit were cited per California Code of Regulations, Title 22.
