Facility Evaluation Report

Licensing Program Analyst Shawna Doucette arrived at the facility unannounced to conduct a complaint investigation. During the course of the investigation LPA observed additional deficiencies. LPA met with Staff Paula Maribel Brizuela.

LPA interviewed staff. LPA reviewed records for R1. Facility did not have all of R1's medications listed on the centrally stored log that are currently being administered. LPA took a photo of R1's centrally stored log.

A copy of this report with plan of correction and appeal rights were provided.
