STATE OF CALIFORNIA - HEALTH AND HUMAN SERVICES AGENCY
This is an official report of an unannounced visit/investigation of a complaint received in our office on
07/22/2025 and conducted by Evaluator Shawna Doucette
COMPLAINT CONTROL NUMBER: 24-AS-20250722112548
Facility staff are not meeting resident diapering needs.
Licensing Program Analyst (LPA) Shawna Doucette conducted an unannounced complaint visit and was granted entry by Administrator Nancy Cudal. LPA explained the purpose of the visit.
LPA interviewed staff and obtained copies of R1's file.
Based on interviews and records review, R1 is not normally incontinent per LIC 602 and staff interviews. Staff interviews indicated R1 became ill and was soiling diapers more than every two hours. Facility staff then sent R1 to the hospital to be evaluated. Staff interviews indicated R1 was changed right before sending R1 to the hospital. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is UNSUBSTANTIATED.
