Facility Evaluation Report
On 4/08/25, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Case Management visit. LPA introduced self, stated purpose of visit and allowed entrance by direct care staff. Administrator, Adonica Vickers (ADM) contacted by telephone and was unavailable to conduct visit with LPA. LPA met with House Manager, Anges Fanoy to conduct visit.
The purpose of this visit is to verify that Staff (S1) is not on the property. Per House Manager, S1 never started employment with facility. House Manager understands that S1 is Excluded and not permitted to be on the grounds at any time.
House Manager disassociated S1 from facility personnel report provided by LPA during visit today’s visit.
An exit interview was conducted and a copy of this report will be e-mailed to Administrator.
