On 12/20/2025, Licensing Program Analyst (LPA) conducted an unannounced Case Management visit, LPA met with House Manager, Idelli Ervin and communicated with Licensee, Adonica Vickers via telephone to conduct visit.

LPA arrived to follow up on a report received in Department regarding R1 and current hospitalization. LPA requested the following documents to be submitted to Fresno Regional Office no later than 12/22/25: Physician's Report (LIC 602), Appraisal/Needs and Services Plan (LIC 625), Identification and Emergency Information (LIC 601), Advance Health Care Directive, medical records available.

No deficiencies cited during Case Management visit.
