Facility Evaluation Report
On 06/22/2026, Licensing Program Analyst (LPA) J. Duarte conducted an unannounced Annual Required inspection visit. LPA arrived, introduced self, and allowed entrance by staff. Staff contacted House Manager Niesha Payton via telephone and she arrived shortly after.
Facility tour conducted with House Manager, Neisha Payton. Facility is a 3 bedroom, 2 bathroom home, currently all bedrooms are private. Facility observed to be clean, odor free, and and the temperature was at 76 degrees F. At the time LPA arrived there was one resident home and per staff, the two other residents were on an outing. All common areas of facility observed to have adequate seating available for residents in care.
Facility tour began in resident bedrooms, all bedrooms observed to have adequate lighting and required furnishings. Resident bathrooms toured, showers observed to have grab bars and shower chairs available. Water temperature measured at 112 degrees F.
Kitchen toured, all knives observed to be locked and secured in kitchen drawer. Facility observed to have a two day supply of perishable and a seven day supply of non-perishable food available. All food in the refrigerator observed to be properly stored and labeled. Medication observed to be locked and secured in hallway cabinet. Facility stores injectable refrigerated medication in a lock box inside the fridge. Medication reviewed and observed to have original labels. The MARs reflect the medication to be administered as prescribed.
Fire extinguisher present and observed to have a service date of 07/23/2025. Carbon monoxide and smoke detectors observed operational during inspection. Last fire drill conducted 03/26/2026, according to facility records.
