Facility Evaluation Report
On 11/17/2025, Licensing Program Analyst (LPA) M. Medina arrived at the facility unannounced to conduct an Annual Required inspection. Administrator, Luis Raya was not available, LPA met with Licensee/Administrator, Arturo Gonzalez to conduct today's visit.
Facility is a 4 bedroom, 2 bathroom home. All resident bedroom are private. Facility observed to clear, odor free, sufficient lighting, and a comfortable temperature. Facility tour began in resident bedrooms. All bedrooms observed to have required furnishings. All common areas observed to have adequate seating for residents in care. Kitchen toured, facility observed to have a 2-day supply of perishable food and a 7-day food of non-perishable food available. Knives were stored and secured in hallway closet. Medications observed to have original labels and to be administered as prescribed. Bathroom toured, fixtures observed operational during facility tour. Water temperature measured at 109 degrees F.
Smoke detectors and carbon monoxide detectors observed to be operational during facility inspection. Fire extinguishers have a service date of 4/01/2025 and last fire drill conducted on 10/17/25 according to facility records. All chemicals are locked and secured in cabinet within the garage.
Outside of facility toured. Facility has outside shaded and seating area for residents. All exits are free of obstruction and no hazards observed.
An exit interview was conducted and a copy of this report provided for facility records. .
No deficiencies observed during inspection visit.
